In Toolbar Extras version 1.3.0 there was a little bug which could led to the display of unwanted PHP notices in some installs.
To get rid of those notices just save the plugin’s settings again: WordPress Settings > Toolbar Extras
There save the “General” tab once, plus the “Smart Tweaks” once.
Then all is back to normal.
The bug was fixed in version 1.3.1.
